The ARC model augments the quantity and quality of patient encounters in various levels of care from hospital based rehabilitation to post-acute facilities. This has resulted in a significant reduction in preventable re-hospitalization rates; in many cases by greater than 50 percent. ARC continues to grow and meet the needs of the changing continuum by providing clinical-based solutions to address issues that most affect health care: clinical coverage, re-admission rates, patient satisfaction, program building, and outcomes.
