**Job Title:** Perm – Physician – Primary Care

**Location:** 174 Edison Road,Lake Hopatcong, NJ 07849

**Shift:** day Shift

**Pay Rate:** $220,000-$240,000/Yearly.

**Submission Requirements**

* TRULY Board Eligible (within 5 years of eligibility) or Board Certified in Family Medicine or Internal Medicine – **REQUIRED**
* NJ license (or willing to obtain) – **REQUIRED**
* DEA Verbal Confirmation (or willing to obtain) – **REQUIRED**
* CSR/CDS Certified (or willing to obtain) – **REQUIRED**
* BLS (or willing to obtain) – **REQUIRED**
* Clean malpractice – **REQUIRED**
* Does the provider require visa support – J1 or H1B? (note in “Additional Submission Details” field) – **REQUIRED**

**Certification Requirements**

* BLS
* Board Certified
* Controlled Substance (CSR/CDS)
* DEA

**State License Requirements**

* New Jersey

Job Type: Full-time

Pay: $220,000.00 – $240,000.00 per year
