**Job Title:** Perm – Physician – Rheumatology

**Location:** 2420 Lake Avenue, Ashtabula, OH 44004

**Shift:** Day 5×8-Hour

**Pay Rtae:** $130,000-$136,451 per year

**Submission Requirements**

* Board Eligible or Board Certified in Rheumatology – **REQUIRED**
* Ohio license (or willing to obtain) – **REQUIRED**
* Provider must within a 40-mile radius or be willing to relocate (note in “Additional Submission Details” field) – **REQUIRED**

* Board Certified
* Board Eligible

**State License Requirements**

* Ohio

Job Type: Full-time

Pay: $130,000.00 – $136,451.00 per year
