Job Description

* Job Type: M-F, Full-time
* Pay: 60-70k annual
* Location: New Hartford, NY

**Job Summary:**

We are seeking a dedicated Dietitian to join our team at a reputable rehab facility. As a Dietitian, you will be responsible for providing comprehensive nutritional care to our residents. Your role will involve assessments, documentation, menu planning, and collaborating with interdisciplinary teams to ensure optimal nutrition for our residents.

**Responsibilities:**

* Conduct quarterly assessments and document the nutritional care of all tube-fed residents.
* Review and intervene in response to significant changes or medical conditions indicated by nursing or identified by the MDS Coordinator.
* Plan, review, and revise cyclical menus and modified diet extensions, ensuring nutritional adequacy.
* Assess and provide follow-up care for residents considered to be at high nutritional risk upon admission or re-admission.
* Document weekly skin assessments using the designated assessment sheets.
* Complete annual assessments with RAPS and MDS and contribute to care planning.
* Participate in dining room meal observation and supervision, including oversight of the diet technician’s meal supervision.
* Develop and implement individualized nutritional care plans, attending interdisciplinary care plan meetings for the assigned caseload.
* Collaborate with medical and nursing staff to provide expert consultation on dietetics.
* Perform all duties in compliance with ethical standards, corporate compliance plan, and applicable laws.

**Qualifications and Requirements:**

* Bachelor’s degree in Nutrition, Dietetics, or a related field. Master’s degree preferred.
* Registered Dietitian (RD) certification required.
* Current state licensure or eligibility for licensure as a Dietitian

Job Type: Full-time

Pay: $60,000.00 – $70,000.00 per year

Experience:

* Clinical Dietitian: 1 year (Preferred)
* Outpatient Diabetes: 1 year (Preferred)

License/Certification:

* current state license as a Registered Dietitian (Required)

* Syracuse, NY: Relocate before starting work (Required)
